Visit Nurse
On-Call, Home Health Department
The Visit Nurse is responsible for providing and evaluating the initial and
on-going need for nursing services in the home. This position is accountable for coordinating home care and
ancillary services to the consumer in order to promote independent living.
Qualifications include:
- Licensed as a “Registered
Nurse” under Wisconsin s. 441.06
- Keep RN license current
- Maintain a valid Wisconsin
driver’s license
- Have means of dependable
transportation
- Able to meet physical demands
of assigned case load (lifting, transferring, etc.)
- Good written and verbal
communications skills
- Prefer one year experience as
a registered nurse in the home care, rehabilitation, medical/surgical, or
related field

Personal Care Workers
Full-Time and Part-Time, Home Care Department
The Personal Care Worker is responsible for providing personal care services
under the supervision of a registered nurse and written “Care Plan” for
consumers in their own homes to assist them in living independently.
Qualifications include:
- Be trained in the provision of
personal care services (Training shall consist of a minimum of 40 classroom
hours, at least 25 of which shall be devoted to personal and restorative care,
or 6 months of equivalent experience.
Training shall emphasize techniques for and aspects of caring for the
population served by the provider.)
- Provide documentation of
required training to the personal care provider for the provider’s records
- Be a person who is not legally
a responsible relative of the recipient under s.49.90 (1) Stats
- Be a person who has not been
convicted of a crime which is directly related to the occupation of providing
personal care or other health care services

Certified Nursing Assistants
Full-Time and Part-Time, Home Health Department
A Certified Nursing Assistant is an one-the-job trained, para-professional worker
who is prepared to assist professional nursing personnel in the care of the
consumer in the home and functions as a substitute for a family member in
providing home nursing care on a part-time basis for the sick and/or disabled
member of the family. The duties of the
Certified Nursing Assistant are planned and assigned by the professional nurse
to meet the client and family situation.
Qualifications include:
- An interest in people
- Tact
- Good judgment in dealing with
those who are ill and disabled
- Neat in appearance
- Practices good personal
hygiene
- History of good, emotional
stability
- Priority is given to the
applicant who has completed a high school education
- Must have successfully
completed a course of training as required by HSS 133.17(4)
- Ability to accept supervision

The Communication Assistant is responsible for providing the means for equal
access to telecommunications services and systems for four groups of people:
deaf, hard of hearing, speech disabled and hearing who use a text telephone
(TTY).
Qualifications include:

Professionally and courteously relaying a variety of call types distributed to
their workstation using a computer and special software

Ability to type at a rate of 60+ words per minute and use accurate spelling when
relaying calls

Sufficient hearing skills to process phone calls

Ability to speak clearly with a variety of tones and inflections from typed
words
